MACHW Training Series: CHW Documentation

This training will focus on the importance of accurate, timely, and professional documentation in community health settings. Key topics will include appropriate note-taking, use of approved terminology, confidentiality and HIPAA compliance, documentation of outreach and encounters, and aligning documentation with organizational and funder requirements.
