UNITE HERE HEALTH is seeking a Bilingual Chinese (Cantonese) Health Promoter (Community Health Worker) for our Boston office. Our ideal candidate is passionate about social justice, working with a diverse, working class population and has experience in at least one of the following fields: healthcare, customer service, community education and/or service sector labor unions.

 

Job Duties

·   Conducts proactive education and advocacy via phone, email, text and in person

·   Meets with, educates and motivates participants regarding participation in Fund programs and care coordination designed to improve health outcomes

·   Recruits participants into Fund self-management programs (individual and group); follows up with participants to ensure maximum participation

·   Coordinates with contracted Case Management partners and community resources to ensure appropriate services are attained for high need participants

·   Meets with participants at their worksite, home, union office and places where they receive medical services

·   Reviews data to determine priority targets for program outreach

·   Promotes COVID-19 vaccine and educates participants about vaccine importance, benefits, and efficacy and addresses hesitancy

·   Promotes other vaccines as needed

·   Educates and assists participants losing coverage to find alternative forms of coverage

·   Identifies, recruits and trains participant leaders to expand participation in these programs through peer-to-peer outreach

·   Documents work and outcomes in CarMa, the Fund’s in-house participant engagement tracking software

·   Provides input to management regarding participant experience and concerns identified through interactions with participants

·   Performs special projects as requested by Fund Management

·   Drives on Fund Business and/or authorized to drive a Fund-owned or leased vehicle

·   Sets goals and achieves measurable results

·   Contributes ideas to plans and achieving department goals

·   Performs other duties as assigned within the scope of responsibilities and requirements of the job

 

Qualifications

Years of Experience and Knowledge

·   2 ~ 3 years of related experience minimum

·   Working knowledge and experience in healthcare, service sector labor unions, healthcare community organizing, healthcare customer service and/or health outreach and education

 

Education, Licenses, and Certifications

·   Associate’s degree or equivalent work experience required

·   Valid driver’s license, car, auto insurance, or ability to use public transportation

 

Skills and Abilities

·   Strong communication skills (written and verbal)

·   Intermediate proficiency in Microsoft Office suite (Word, Excel, Outlook)

·   Intermediate proficiency computer skills

·   Fluency (speak and write) in Chinese (Cantonese)
