Community Health Improvement Specialist

We are hiring multiple Community Health Improvement Specialists who will each support one primary community in Chelsea, Revere, Dorchester, Roxbury, Mattapan, Mission Hill, Hyde Park, Jamaica Plain, Roslindale, Lynn, or Salem. This role requires deep familiarity with the strengths, cultures, and needs of the communities served. Candidates with lived experience in or strong connections to these communities are encouraged to apply.

This position focuses on outreach, education, and navigation related to two of MGB’s identified priority conditions: colorectal cancer and cardiometabolic disease. Specialists provide blood pressure screenings, deliver basic health counseling, and support individuals in connecting to services that promote prevention, early detection, and long-term health.

Qualifications
High school diploma or equivalent is required; Associate or Bachelor’s degree is preferred
2 -3 years of experience in community outreach, patient navigation, health education, or other public health roles is required
Experience conducting health screenings or participating in health promotion activities is preferred; training will be provided
EMT or Medical Assistant experience strongly preferred, particularly in patient-facing or clinical support roles.
