Community Health Worker – School Based Health Center

The School Based Health Center Community Health Worker (CHW) is part of Heywood Healthcare’s School-Based Services team and is located at the Murdock School Based Health Center in Winchendon. This role works closely with the onsite Nurse Practitioner and Behavioral Health Clinician to support student health and access to care. Responsibilities include assisting with medical coordination, maintaining records in the electronic medical record, helping students and families access community and healthcare resources, and connecting with the school community.

This position is 30 hours per week during the school day and follows the school calendar (42 weeks per year), while still offering benefits and paid time off. The CHW is part of a larger, supportive SBS team across multiple schools and plays an important role in helping students access medical and behavioral health services and stay healthy and successful in school.
