Who are CHWs

Community Health Workers (CHWs) are front-line public health professionals; they are trusted by members of their communities to share information and facilitate access to health and social services.

With a deep understanding of their community based on lived experience, CHWs serve as a bridge between social services or healthcare systems and their neighbors while ensuring language access and culturally appropriate care. They are skilled outreach and referral specialists, community health educators, health coaches, patient navigators, peer advocates, and more who work to reduce social and racial inequities in health.
