Community Nurse Home Care is hiring a Community Health Worker for our Care Transitions Program. Part-Time

The Community Health Worker (CHW) plays a key role by providing social, behavioral and resource support to patients as they navigate the health care system. The CHW is a member of the primary care team. Partners closely with care team, patients, and outside organizations. The CHW is responsible for increasing patient’s access to health care and community resources/social services, facilitating communication between patient and care teams; and providing culturally appropriate health education and self-management support.

Key Responsibilities

Coach patients in effective management of their chronic health conditions and self-care
Facilitate referrals to community-based resources.
Assist patients in identifying the barriers to their health that they prioritize.
Assist patients in care plans and instructions; help patients maintain/develop health management plans and goals.
Document activity, care plans and outcomes in an effective manner consistent with CNHC policies and procedures.
Develop on going follow up, basic motivational interviewing and goal setting with patients.
Help patients in identifying and utilizing community resources.
Facilitate communication and coordinate services between providers, work collaboratively and effectively within multidisciplinary teams.
Build and maintain positive working relationships with patients, care team, providers, supervisors and co workers.
Continually expand knowledge and understanding of community resources, services and programs provided.
Identify and apply appropriate role definition and skilled boundaries.
Support Tele Health Program – Tele Health Installs and teaching
Requirements/Qualifications

High school degree or equivalency
Community Health Worker License
Any combination of 3 years of health/social services experience preferably in a health care or community based organization.
Creativity, flexibility, sound judgement and ability to take initiative
Strong communication and writing skills
Excellent time management and organizational skills
Ability to work effectively as a team member in a complex team based environment
Excellent interpersonal skills and demonstrated ability to interact professionally with culturally and educationally diverse staff and clients.
Bilingual/bicultural encouraged to apply
Computer competency with an Electronic Medical Record and Microsoft Office
Positive attitude and open to changing environment.
