Grant-funded – 24 hours/week. Non-Exempt. Some evenings and weekends required.

 

REACH LoWELL COVID/Flu, funded by the Centers for Disease Control and Prevention seeks to eliminate disparities among the Southeast Asians, Hispanic/LatinX communities, and other minority populations. Outreach efforts will target Brazilian, Latinos, Africans, Cambodians, and other communities at risk of higher rates of health disparities. Outreach efforts will be coordinated with various community-based agencies, faith-based organizations, and other community programming venues in addition to Lowell CHC departments and programs.

 

The Lowell Community Health Center (Lowell CHC) REACH LoWELL COVID/Flu program will hire a part-time Khmer and/or Portuguese speaking Community Health Worker to provide COVID/Flu outreach education, distribute health promotion materials in English, Khmer, Portuguese and/or Spanish. The CHW will conduct extensive COVID/Flu outreach in their community, with the goal of enhancing awareness of COVID/Flu related resources to providers and other agencies. This position requires an individual committed to health equity and social justice that is adaptable in a variety of community settings.

 

RESPONSIBILITIES include the following. Other duties may be assigned.

 

Community Health Worker

§  Conduct extensive outreach in their community, with the goal of enhancing awareness of resources, more so related to COVID/Flu vaccine, education and awareness

§  Work collaboratively with REACH LoWELL COVID/Flu Coordinator to support program deliverables

§  Provide translation and interpretation support in target language in alignment with REACH LoWELL program deliverables

§  Promote health education in alignment with REACH LoWELL COVID/Flu strategies

§  Be knowledgeable about community resources appropriate to the needs of patients/families and provide services such as arranging transportation, referrals to community programs, setting up appointments when needed and connect to financial support.
Collaborate with community agencies to coordinate resources/referrals and network with community leaders

§  Support patients and their families to navigate and access community services, other resources, and adopt healthy behaviors

§  Attend and actively participate in appropriate trainings and meetings relevant to REACH LoWELL COVID/Flu strategies

§  Provide advocacy for initiative priorities through a strong presence at community meetings, events and activities.

§  Participate at various community events and Community Health education efforts, including community screenings, Community education groups and Health Center’s Advisory groups, as needed.

§  Represent Lowell CHC mission and programs at meetings, conferences, on the phone and in person

§  Prepare reports in accordance with Lowell CHC procedures and ensure that the reports are submitted in a timely manner as required by REACH LoWELL COVID/Flu Program Director and participate in staff meetings

§  Assist with required data collection and evaluation reporting as directed.

§  Provide evaluation support to REACH LoWELL COVID/Flu team

§  Reports to Lowell CHC REACH LoWELL COVID/Flu Outreach Coordinator

§  Perform other duties as requested to support the mission of REACH LoWELL and Lowell CHC

Maintain standards that ensure quality of care in the delivery of program services and compliance with state and federal guidelines. Grant funded position, available pending grant availability.

 

QUALIFICATIONS:

Required:

o   Lives in or familiarity with the target community and feels connected to that community

o   Community organizing skills

o   Excellent verbal and written communication skills

o   Strong presentation skills

o   Two to four years experience in a community based setting or health related position

o   Demonstrated experience working with diverse cultures

o   Detail oriented, with strong organizational skills and multi-tasking abilities

o   Ability to work independently with minimal supervision and as part of a team

o   Must have reliable transportation

o   Proficient computer skills in MS Office

Preferred:

Community Health Worker Training Certificate
At least 2 years education in Public Health, or equivalent
Knowledge of the US health care system and medical terminology
Ability to read and write in Khmer and/or Portuguese
LANGUAGE SKILLS Bilingual in English and Khmer and/or Portuguese

 

SUPERVISORY RESPONSIBILITIES: No

 

SALARY & HOURS: Subject to Lowell CHC’s discretion; Flexible evenings and weekends.
