REACH LoWELL, funded by the Centers for Disease Control and Prevention seeks to eliminate disparities among the Southeast Asians and Hispanic/LatinX communities as it relates to diabetes. REACH LoWELL includes culturally tailored health promotion activities, systems change to increase physical activity and access to healthier foods in the community; and increased community-wide education, with the aim of increasing knowledge of health care system navigation and access to community-based resources.

The Lowell Community Health Center (Lowell CHC) REACH LoWELL program will hire a Community Health Worker to provide outreach education, distribute health promotion materials in English and Spanish. The CHW will conduct extensive outreach in their community, with the goal of enhancing awareness of resources, and to providers in order to increase their sensitivity to cultural health beliefs and to increase cross referrals. Collaboration with community agencies, coordinate resources/referrals and network with Community leaders. This position requires an individual committed to health equity and social justice, and adaptable in a variety of community settings.

Requirements
RESPONSIBILITIES include the following. Other duties may be assigned.

Community Health Worker

? Conduct extensive outreach in their community, with the goal of enhancing awareness of resources

? Support patients and their families to navigate and access community services, other resources, and adopt healthy behaviors

? Work collaboratively with REACH LoWELL Coordinators to support providers through an integrated approach to disease management and community outreach

? Promote health education in alignment with REACH LoWELL strategies

? Provide translation of health education information as needed in Spanish

? Be knowledgeable about community resources appropriate to the needs of patients/families and provide services such as arranging transportation, referrals to community programs, setting up specialist and PCP appointments when needed and connect to financial support

? Collaborate with community agencies, coordinate resources/referrals and network with Community leaders

? Attend and actively participate in appropriate trainings and meetings relevant to REACH LoWELL strategies

? Provide advocacy for initiative priorities through a strong presence at community meetings, events and activities.

? Participate at various community events and Community Health education efforts, including community screenings, Community education groups and Health Center’s Advisory groups, as needed.

? Represent Lowell CHC mission and programs at meetings, conferences, on the phone and in person

? Prepare reports in accordance with Lowell CHC procedures and ensure that the reports are submitted in a timely manner as required by REACH LoWELL Program Director and participate in staff meetings

? Assist with required data collection and evaluation reporting as directed.

? Provide evaluation support to REACH LoWELL team

? Reports to Lowell CHC REACH LoWELL Program Manager

? Perform other duties as requested to support the mission of REACH LoWELL and Lowell CHC

Maintain standards that ensure quality of care in the delivery of program services and compliance with state and federal guidelines.

Grant funded position, available pending grant availability.

QUALIFICATIONS:

Required:

o Lives in or familiarity with the target community and feels connected to that community

o Community organizing skills

o Excellent verbal and written communication skills

o Strong presentation skills

o Two to four years’ experience in a community based setting or health related position

o Demonstrated experience working with diverse cultures

o Detail oriented, with strong organizational skills and multi-tasking abilities

o Ability to work independently with minimal supervision and as part of a team

o Experience working with grant funded programs

o Must have reliable transportation

o Proficient in remote meeting platforms (Teams, Zoom etc.)

o Proficient computer skills in MS Office

o Background or knowledge of project coordination

Preferred:

o Bachelor’s Degree in Public Health, Health Education or related field

o Community Health Worker Training Certificate

o Knowledge of medical terminology
