Position Title: Community Health Worker: full time (40 hours) in a 3 year grant funded position
Project Title: Identify 2500 patients annually with latent tuberculosis infection and treat 90% of those identified

The Community Health Worker is part of an integrated and interdisciplinary care team at the Lynn Community Health Center. The CHW will help in identifying patients at high risk for developing tuberculosis. Locations will include LCHC and affiliates, schools in the City of Lynn, shelters and other facilities that work with homeless and indigent clients, and places yet to be determined.
The goal is to identify 2500 patients the first year of the grant who are at high risk of developing tuberculosis. These patients will be tested using IGRA (Quantiferon TB Gold Blood Test).
If IGRA is positive, indicating that the patient has been infected with the tuberculosis bacteria, a chest X-ray will be performed to assess for active TB disease. If active TB disease in NOT identified, the patient will be recommended by the provider for treatment. Those with TB disease will be referred to a specialist.
The CHW will assist the Project Manager and TB Clinic Manager in identifying patients with latent TB infection, ensuring the clients’ needs and needs of the TB Clinic are met. He/she will assist with appointments, prescription refills, and testing of the clients by coordinating services and facilitating access. He/she will also be responsible for internal and external program reporting, documentation, assist in data collection, and actively participate in ongoing QI activities.
Additionally, the CHW educates, advocates, and coaches patient to complete treatment and assists them in this goal. He/she will work closely with the RN manager and complete face to face and telephone contact with patient.
This is an exciting opportunity to work with LCHC TB Clinic, the CDC, and Mass DPH in developing a system to eliminate TB in the United States.
Qualifications: Bachelor’s degree in health science, social work, human services or public health related field
Bi-lingual in Spanish
Strong organizational, interpersonal, time management, and problem solving skills with attention to detail.
History of good attendance and positive work attitude
Ability to work with and relate to individuals with diverse backgrounds and experiences
Excellent communication skills
Computer literate, preferably with knowledge of EPIC EMR
Ability to work both independently and as part of a team
Experience and Training:
2 or more years of experience as CHW or case manager in a community health setting
Training and or certification as a CHW preferred
Experience providing health education
TB related knowledge or experience helpful
The essential duties of this position present risk of exposure to airborne infection, therefore, must have an understanding of airborne disease transmission as well as use of PPE (Personal Protective Equipment).
Must have updated TB testing and chest X-ray and symptom questionnaire if TST or IGRA is positive
Immunizations must be up to date and are available free of charge at LCHC
